University of Alaska Transportation Safety Guide

This document establishes guidelines that must be followed for safe operation of a University vehicle (this includes cars, trucks, boats, snowmachines, ATVs, motorcycles, private aircraft, etc.). All employees, volunteers, or affiliates seeking to operate a University vehicle, must read and understand this guide. Employee Drivers
Step 1: Take Driver Training

All employees seeking to operate a University vehicle are required to take Driver Training. Navigate to the EHSRM Other Basic Training Page, download and read the PowerPoint, and then take the Quiz. Make sure to understand the content of the PowerPoint before taking the quiz, as a score of 100% is required to complete training. Please forward the training completion email and/or computer screenshot, showing training completion, to ua-museum-operations@alaska.edu for completion verification.

Note: EHSRM recommends that this training be retaken every four years.

 

Step 2: Fill out a Driving Form

Before operating a vehicle, all employees are required to fill out a Category 1 or Category 2 Driving Form and attach all of the required documents listed on the form. Navigate to the EHSRM Travel & Driving Forms Page and scroll down until you see a link titled "Category 1 & 2 driving forms". Clicking on this link will download both Category Driving Forms. Employees are only required to fill out one of the Category Driving Forms found on the downloaded PDF, either the Category 1 Driver Form or the Category 2 Driver Form. Do not fill out both. Submit the completed form to the employee's Supervisor for final approval.

Note: These forms need to be filled out annually.

Volunteer and Affiliate Drivers
Step 1: Take Driver Training

All volunteers and affiliates seeking to operate a University vehicle are required to take Driver Training. Step 2: Fill out a Driving Form

Step 3: Fill out a Driving Waiver

All volunteers and affiliates seeking to operate a University vehicle are required to fill out a Driving Waiver Form and attach all of the required documents listed on the form. The list of required documents include: a completed Category 1 or Category 2 Driving Form (step 2 above), a DMV driving record for all states the volunteer or affiliate resided in the last 3 years, and evidence of completion of Driver Training (step 1 above). There are additional documents and training required for volunteers and those that will be driving with minors in a University vehicle. Navigate to the EHSRM Travel & Driving Forms Page and scroll down until you see a link titled "UAF Driving Waiver". Clicking on this link will download the waiver form. Submit the completed form to Brandon Cruz of EHSRM for final approval.

Use of Other Modes of Transportation

All employees, volunteers, or affiliates seeking to operate a boat, snowmachine, ATV, motorcycle, private aircraft, or other non-conventional vehicle (e.g. not a car or truck) are required to fill out a Modes of Transportation Form. This form asks for training and years of experience each driver has for the vehicle, the safety equipment that will be included on the vehicle, and a list of potential risks and ways of mitigating those risks when operating the vehicle. Navigate to the EHSRM Travel & Driving Forms Page and scroll down until you see a link titled "Modes of Transportation form". Clicking on this link will download the form. Submit the completed form to to the employee's Supervisor first for approval, then send the form to Brandon Cruz of EHSRM for final approval.

Use of Private Aircraft and/or Watercraft

In addition to filling out a Modes of Transportation Form, those employees, volunteers, or affiliates seeking to use a personal aircraft or watercraft for University travel must fill out a Private Aircraft Agreement or Private Watercraft Agreement. Similar to the Modes of Transportation Form above, these agreements asks for the training and years of experience each driver has for their aircraft or watercraft. Navigate to the EHSRM Travel & Driving Forms Page and scroll down until you see the links titled "Private Aircraft Agreement" or "Private Watercraft Agreement". Clicking on either of these links will download the agreements. Submit the completed form to to the employee's Supervisor first for approval, then send the form to Brandon Cruz of EHSRM for UAF approval. EHSRM will then forward the agreement to University of Alaska Statewide Risk Management for final approval.

Vehicle Rentals
