On campus housing requirements
Before arriving on campus
All newly admitted students living in campus housing must provide immunizations which can be uploaded to our Patient Portal. Instructions for Patient Portal and uploading documents. Students can also update medical information, view and print their immunization records and see other important information and outreaches available. Sign in using UA student id# and password. If you have or need an exemption form, please see our forms section and upload your forms to this patient portal. All documents can be uploaded under the Immunization Upload.
Please submit the following forms when uploading immunization records in response to Covid-19 (these forms are required in order for us to confirm compliance with Residence Life):
Copy of your insurance Card (front and back)
Appointments for Covid Testing, Covid Vaccination (Janssen & Pfizer) and Required Immunizations for housing can be made through the Patient Portal.
Immunizations and tuberculosis testing
Prior to moving into UAF housing, you are required to submit immunization records showing that you have had the following:
If you do not have access to these records, you can check with your high school or you can obtain these immunizations and TB testing at the Student Center for Health and Counseling or at a clinic or pharmacy in the community. If done in the community, please upload immunization records to Patient Portal, or fax to 907-474-5777 or bring the documents to the Student Health and Counseling Center or send the documents to:
