Make a report
Below you will find links to submit an online report for the most common issues handled by the Office of Rights, Compliance and Accountability (ORCA). If you do not see a form related to your concern, or if you prefer not to use an online form, you may make a report by calling us at 907-474-7300, emailing us at uaf-orca@alaska.edu or visiting us on the third floor of Constitution Hall (office hours are provided at the bottom of this page).
ORCA is not an emergency response department, if you are reporting an emergency, call UAF Dispatch at 907-474-7721 or dial 911.
Submitting anonymous reports may inhibit the University's ability to fully address the concern.
Reports of civil rights violations (discrimination, sexual misconduct, etc.) may also be made to outside agencies. A list of agencies and their contact information can be found on the UA Notice of Nondiscrimination. Reports may be filed simultaneously with UAF and external agencies.
Report concern for a student (CARE)
You should report behaviors or situations that may impact a student’s ability to be successful at UAF or pose a safety/wellbeing concern. Examples of reportable concerns include:
- Loss of a family member or friend
- Sudden changes to self-care routines or hygiene
- Threatening or intimidating behavior
- Disruptive classroom behavior
- Emotional distress, such as anxiety or depression
- Suicidal ideation
- Prolonged illness or injury
- Hospitalization
- Loss of reality and/or disjointed thinking
- Isolation from peers
- Excessive absence from class or activity
Early intervention is essential in student care. Report concerns as soon as you suspect there may be an issue.
Report student misconduct
All instances of student misconduct should be reported for evaluation. Prohibited behavior is detailed in the Student Code of Conduct. Examples of reportable misconduct include:
- Academic dishonesty (cheating / plagiarism)
- Forgery / falsification
- Property damage
- Theft
- Harassment
- Endangerment or physical harm to others
- Disruptive or obstructive behavior
- Failure to comply with university directives
- Misuse of drugs or alcohol

Report discrimination
Discrimination refers to being adversely treated or affected, either intentionally or unintentionally, in a manner that unlawfully differentiates or makes distinctions on the basis of the individual’s legally protected status. Examples of legally protected statuses include:
- Race
- Religion
- Color
- National origin
- Citizenship
- Age
- Sex
- Physical or mental disability
- Status as a protected veteran
- Marital status / changes in marital status
- Pregnancy, childbirth or related medical conditions
- Parenthood
- Sexual orientation
- Gender identity
- Political affiliation or belief
- Genetic information

Report sex- or gender-based misconduct (Title IX)
Sex- and gender-based misconduct is a serious violation, and for that reason, most university employees are considered mandatory reporters for behavior occurring within university programs or activities. Examples of behaviors which must be reported include:
- Sexual harassment
- Quid-pro-quo arrangements / proposals
- Sexual assault
- Dating violence
- Domestic violence
- Stalking

Report an accessibility barrier (digital or physical)
If you encounter a facility, website, policy, etc. at UAF that prevents full access or participation, please report it to us. Examples of barriers include:
- Inaccessible course materials or websites
- Inaccessible walkways
- Inoperable power assisted doors
- Inoperable elevators / lifts

Report a concern about a course (for students)
If you have a concern about how a course is being delivered, you can report it here. Examples of concerns include:
- Failure to follow the syllabus
- Problematic classroom policies
- Misunderstandings with instructors / staff
- Instruction delivery issues

Report hazing
Hazing is defined as an intentional, knowing, or reckless act committed by a person (whether individually or in concert with other persons) against another person or persons regardless of the willingness or acquiescence of such other person or persons to participate, that:
- is committed in the course of recruitment for, initiation into, an affiliation with, or the maintenance of membership in, a student organization, team, or similar group-affiliated activity; and
- causes or creates a risk - above the reasonable risk encountered in the course of participation in university activities or the organization - of physical or psychological injury.
Report a crime
Criminal activity should be reported to the University Police Department by calling dispatch at 907-474-7721. In the event of an emergency, call 911.
If you do not want to report the crime to police, but only wish to report a crime for counting in the university’s annual crime statistics under the Clery Act, you may use this form. Names are not required on the form, but a description of the criminal activity and a detailed location will assist us in classifying it correctly for annual statistics.
UA confidential hotline
The University of Alaska maintains a confidential hotline through EthicsPoint. Reports may be made online or by phone. Hotline reports do not go directly to ORCA, but a report might be referred to our office if it involves matters of student care, student conduct, or civil rights violations.
