Qualify for $600 wellness incentive through April 30

The Healthyroads program, provided to employees, spouses and partners enrolled in
the UA Choice health plan, is back with an exciting new wellness incentive — a chance
to earn up to a $600 rebate on fiscal year 2016 health plan contributions.
To earn your incentive, complete your personal health assessment, get a biometric
screening and earn five credits through a variety of healthy activity options. Be
sure to complete everything on or between Nov. 1, 2014, and April 30, 2015.
The wellness rebate of up to $600 for you and $600 for your spouse or financially
interdependent partner will be paid out in biweekly payments or “credits” on your
paycheck, just like you pay for health insurance. If you’re a 12-month employee, you’ll
get 26 credits on your paychecks. If you’re a nine-month employee, you’ll get 19 credits.
Wellness visits, routine exams and immunizations covered by the UA Choice health plan
through Premera or VSP since July 1, 2014, will be automatically credited to your
account.
