The Division of Student and Enrollment Services' Early Warning Committee provides extra assistance and coordination to students that may be having difficulties--whether personal, financial, or academic. If you have a student (or students) who are having difficulties in your class, or you feel might need some additional assistance related to attendance or achieving satisfactory grades, please complete the following form as best as you can and click on the "submit" button (don't worry about having all the information, just complete as much as you can).
