  • Please note that if you do not have access to submit internal CRCD courses, please email Crystal Frank at cafrank@alaska.edu requesting access.

Curriculum Forms

Academic Course and Degree Procedures Manual - The UAF Governance website link has all the updated curriculum forms:

  • Format 1 - Trial Course or New Course Proposal Form 
  • Format 2 - Change Course (Major) and Drop Course Proposal 
  • Format 2A - Change Course (Minor) 
  • Format 2B - Course Compression Format
  • Format 3 - New Degree Program Request (include the following forms)
    • Board of Regents Summary Form
    • Resource Commitment Form
    • The submission of a draft prospectus is required along with the Format 3 request.  Below are instructions and a template, along with a sample for reference.
