Class scheduling forms
The following are PDF forms. You will need Adobe Acrobat Reader to view them. Each form can be printed and filled out manually, or they can be filled out online, printed and then signed before delivering to the Registrar's Office.
- Addition to the Schedule (PDF)
Additions to the published schedule will be accepted on this form only. Complete the form (including required signatures) and deliver to the Registrar's Office in Signers' Hall. The completed form may also be faxed to (907) 474-7097. Please note that classroom space is limited, especially during prime times, and will be assigned on a space available basis.
- Change of Schedule (PDF)
Use this form to request a change of days and times and/or room, to change the course maximum, instructors, dates, etc. Department head and dean signatures are required for certain changes. Complete the form and deliver it to the Registrar's Office. The completed form may also be faxed to (907) 474-7097. Please note that classroom space is limited, especially during prime times. Space may not be available for your first choice of time and/or building.
