Mid March 2014Unit Coordinators report quantities of forms needed to the campus wide coordinator at extension 6634 or email Brandi Marrero at bmarrero2@alaska.edu
Date dependent on course end date, variesSURVEYS COMPLETED IN CLASS. The college or unit coordinators collect packets, organize responses, and prepare the Instructor Information Forms (Header Sheets) for each course surveyed. 
By August 29, 2014DEADLINE: Forward completed forms and header sheets to the Provost's Office at box 7580 (attention: Brandi Marrero) 
Early SeptemberForms sent to University of Washington for scanning and processing 
Late SeptemberScanned results from University of Washington are distributed

Mid October 2014Unit Coordinators report quantities of forms needed to the campus-wide coordinator at ext 6634 or bmarrero2@alaska.edu. Forms will be sent to the unit upon request.
