At UAF, it is mandatory that the Instructional Assessment System
survey forms be distributed in all courses/sections with the following
exceptions:
- The course has less then five students enrolled in it
- The course is less than three weeks in duration
- The course is a linked laboratory class
- The course is independent study or individualized instruction (practica,
internship, study abroad, etc.)
Note: Lab
classes and classes with individualized instruction may be evaluated
at the instructor's discretion; however independent study sections (_97,
_98, and _99) will not be evaluated.
More Than One Instructor
If there is more than one instructor for a class, or if a TA is doing
a considerable amount of teaching, separate survey forms must be completed
for each - this includes the Instructor Information (header) sheets.
Step by Step Process

Campus coordinator makes copies of reports and distributes 2 copies and 1 original to colleges/schools for filing. One original is retained in Provost's Office files, one original (a copy) is sent to faculty member, one copy (of original) is put in official academic file in college/school office, and one copy is given to the department chair. |

Completing the Forms
Survey forms shall be completed with a No. 2 pencil. Instructors should allow 15-20 minutes of class time during the evaluation period for students to complete the forms. Instructors are expected to leave the class while the students complete the survey. Instructors ARE NOT ALLOWED to handle complete survey forms in any way. Please be sure that a student courier returns forms to one of the designated drop-off points listed on the courier instructions.
Tips for Processing Forms
- If forms come to you wrinkled, torn, dirty or scribble on put in packs "as is". Do not doctor these up.
- No need to redo forms that are completed in pen.just put in the pack.
- Please don't use paper clips or staples to keep course packs together.
- Cannot mix form types for a course---if there are stray ones in a course
pack these DO need to be pulled out.
- Forms dated 1995 and 1996 are the current forms in use for all forms
other than G. The G form to use should be dated 1999. DO NOT use
anything prior to these dates.
Optional Questions
A faculty member can prepare optional questions for the back side of
each form. Please follow the information on the following web site to
prepare a template: http://www.washington.edu/oea/services/course_eval/forms/template.html. Pull out yellow comment sheets - give to Faculty Member after grades have been submitted.
Processing time frame:
There is no time limit (within reason) as to when forms can be sent to
UW for processing. I will take forms at any time and send to UW, likewise
survey results can be added to the official academic file at any time.
Forms that are late are processed separately from forms in the current
semester so they need to be clearly marked with the semester and year.
Because a separate report must be generated (and this means more money)
please reserve this courtesy for out of the ordinary circumstances only.
This information is primarily intended for the Rural Campuses or distance
delivered courses due to the constraints in sending/receiving the survey
forms.

PLEASE READ THE FOLLOWING STATEMENT TO THE CLASS
UAF was directed by the Board of Regents to develop a process to obtain Student Opinion of Instruction, and, in 1980 the UAF Faculty Senate passed a resolution adopting a university-administered process. The resolution states, “the results of evaluations are to be used to aid curriculum improvement, to aid in improvement of teaching by individual faculty, for documentation for faculty promotion and tenure, and to aid students in course selection.” Because of the importance of your opinion, please mark your responses carefully. We encourage you to take this opportunity to provide your opinion on the survey forms as well as an opportunity to provide written feedback directly to your instructor on the yellow comment sheets. The yellow comment sheets are not read by anyone other than the instructor. Survey reports are available for review in the Rasmuson Library and at the ASUAF office. The Office of the Provost, in conjunction with each College/School, administers the process. |
