User Applications
Authorized
User - This form must be completed by the Principal
Investigator. The RSO will contact you once the application has
been received to provide you with additional materials including
the Radiation Manual and Red Book and to let you know what additonal
requirements you need to complete before your application can be
approved.
Supervised
User - This application must be completed by anyone
working with radioactive sources or materials under the direction
of an Authorized User. This includes graduate and undergraduate
students, research staff or volunteers. The RSO will contact you
once the application has been received to provide you with additional
materials and to let
you know what additonal requirements you need to complete before
your application can be approved.
User Exams: These
exams should only be taken once you have contacted the Radiation
Safety Officer (see contact information), received the Radiation
Manual and Red Book, and have had the opportunity to study the
material. The exams are open book.
Authorized
User Exam - Completion
of this exam is required before you can be designated as an Authorized
User
Supervised
User Exam - Completion
of this exam is required before you can be designated as a Supervised
User.
