Marketing and Communications video shoot request form

NOTE: Only a very limited number of video requests can be filled. Shoots need to be usable by M&C and fit into a timeframe that doesn't conflict with other commitments. Please make your request at least two weeks prior to an event.

To collect the model releases (PDF) either in advance, at the event or afterwards, and provide them to M&C.
To use photo credit sent with the video(s).
Use of the UAF logo is required to use the video(s) sent.
Video(s) provided are protected by copyright.

Requests are reviewed individually, and a reply will be sent within four business days.
