Registration & Application Materials

Biosafety Registration - use this form to register all uses of recombinant DNA, select agents and/or infectious agents.

Modifying an Approved Registration: Submit the revised Registration Form to fyori@uaf.edu. Put "IBC Modification Request" in the subject line and provide a brief summary or description of the changes you are requesting in the text of your email.

Continuing Review - Use this form to renew or close an active Biosafety Registration. This form must be submitted annually to keep the registration active.

Quality Improvement Program

QI Report - this form may be submitted by anyone with specific concerns about UAF activities involving recombinant DNA, infectious agents, or select agents.

Material Transfer Agreements

Material Transfer Agreements are frequently needed to obtain or ship material regardless of the source or destination. All MTAs must be reviewed and signed by the Director, UAF Office of Technology Transfer. To facilitate transfer of biological material the Uniform Biological Material Transfer Agreement (UBMTA) may be used by employees of the University of Alaska. Materials can be transferred under the terms of the UBMTA upon execution of an Implementing Letter for the particular transfer. Contact the UAF Office of Technology Transfer for more information.
