The Data Request form is used to obtain data from our institutional database for official UAF purposes. For invitation lists, please include the Invitation List form to request specific groups of constituents. Email your completed form(s) to Advancement Services at UAFaddress@uaf.edu or fax to (907) 474-1975. Please allow 2 weeks for processing all requests. If the information provided is to be used for fundraising purposes, you must send your fundraising materials to the Development Office for review and acceptance prior to submitting a data request.
