Medical Consent for Release of Information
This form is necessary for our Health and Counseling Center to have on file if your student needs you to have access to their medical information or for our professional staff to discuss medical information with a parent.
The FERPA consent form that parents sign at the Business Office to have access to student records IS NOT a medical consent form which is required by federal law. The HIPAA (Health Insurance Portability and Accountability Act) of 1996, requires us to protect personal health information. Students 18 years of age and older must fill out a MEDICAL consent for release of information form at the Student Health and Counseling Center in order for our staff to communicate with parents.
