Students can obtain vaccinations here at the Health Center after they arrive. We offer PPDs, MMR, tetanus, Meningitis and HPV if they need them.

Medical Consent for Release of Information
This form is necessary for our Health and Counseling Center to have on file if your student needs you to have access to their medical information or for our professional staff to discuss medical information with a parent.
The FERPA consent form that parents sign at the Business Office to have access to student records IS NOT a medical consent form which is required by federal law. The HIPAA (Health Insurance Portability and Accountability Act) of 1996, requires us to protect personal health information. Students 18 years of age and older must fill out a MEDICAL consent for release of information form at the Student Health and Counseling Center in order for our staff to communicate with parents. Student Health insurance policy information for the new academic year will not be available until mid July. We do know, however, that the premiums will increase because the benefits will increase according to government mandate.
Students should be prepared to show proof of insurance if asked by the Business Office. The only insurance Student Health files is the school's insurance plan.
