1. Choose the appropriate form. (By Mail or Fax: Print the mail order form.) (Online: Use our online order form for free publications and the secure form for paid publications.)
2. Fill in your name, mailing address, and date.
3. Note the publications you would like. Include the publication number (as listed in our catalog or on our website), quantity of each publication number, the price per copy, and the total price. If the publications are listed FREE, write FREE in the price column.
4. Videotapes and DVDs may be checked out on loan for two weeks. If you wish to purchase a videotape or DVD, list price and order as above.
5. Videotapes and DVDs for loan will not be mailed outside Alaska.
