Contacts & Application Materials
Applications for hosting a Japanese youth or chaperone are accepted all year. The deadline for hosting the following July is usually in the spring. Please apply early so you can begin corresponding with your Japanese delegate. If you have any questions about filling out an application, or during the application process, please feel free to contact the Alaska State Exchange Coordinator Tony Nakazawa:
If you are applying to host in the Fairbanks/North Pole area or nearby communities, contact Jill Holmgren at firstname.lastname@example.org or call 455-6987.
Please return your application to your local 4-H office or contact Jill or Tony for a mailing address. When your completed application has been received, an exchange program representative will contact you. The program representative will be happy to answer any questions you may have, will get to know your family so they can make the best possible match between the host youth and the Japanese youth, and will provide you with additional resources in preparation for your hosting experience.
The complete application consists of the following:
- Application and family picture. If you don't have a photo with everyone in it, it is fine to use several photos.
- Confidential references from two people who know you.
- Disclosure Form. Please print one copy for each person in the home that is 18 years or older. This form is confidential and will be destroyed once the paperwork has been processed.
Note: You will need to have Adobe Reader installed on your computer in order to download and print these forms
