Application Materials for 2012

This application package may be used to either host a Japanese youth delegate, or a Japanese chaperone. If you have any questions about filling out an application, or during the application process, please feel free to contact the Alaska State Exchange Coordinator Tony Nakazawa:

Once you have completed the forms, please return them to your local 4-H agent, the Alaska state exchange coordinator,  or to the statewide 4-H office in Fairbanks (attention Deb Jones, 4-H statewide director). Once a complete application package has been received, you will be contacted by a Labo program representative.  The representative will have some additional questions for you so that the best possible match is made between the host youth and the Japanese delegate. The program representative will also be happy to answer any other questions you may have and to provide you with additional resources in preparation for your hosting experience. 

The complete application consists of the following:

  1. Application and family picture. If you don't have a photo with everyone in it, it is fine to use several photos.
  2. Confidential references from two people who know you.
  3. Disclosure Form. Please print one copy for each person in the home that is 18 years or older. This form is confidential and will be destroyed once the paperwork has been processed.
